Procuring Your Medical Records

Patients have the right to access their medical records. To request a copy of your records from Indira Super Speciality Hospital:

Online Request (Patient Portal)

The fastest way to access lab results, radiology reports, and discharge summaries is by logging into our secure Patient Portal.

Physical Request

You can request hard copies of your records by visiting the Medical Records Department (MRD) on the ground floor.

• You will need to fill out a Medical Record Request Form.
• Bring a valid, government-issued photo ID.
• If you are requesting records on behalf of a patient, you must provide a signed authorization letter from the patient and your own ID.

Note: Processing physical record requests may take up to 48-72 hours. Nominal printing charges may apply for voluminous records.
